Initial Passport Requirements
What documents do I need to get my U.S. Passport?
Primary Citizenship Document(must submit one):
- A Certified, Government issued Birth Certificate
- A Consular Report of Birth Abroad
- A Naturalization Certificate or Certificate of Citizenship
- A Previously Issued Passport
If you do not have any of the Primary Citizen Documents then you must submit two Secondary Documents:
- Early Public Records (Hospital Birth Certificate or Early School Records)
- Delayed birth Certificate
- Letter of no record
Along with Proof of Citizenship, you will also need Proof of Identity using one of the following forms of ID:
- Drivers License
- Military ID card or current Government issued ID
- Previous Passport
- Naturalization Certificate
If the above forms are unavailable, you may need to submit several of the following Secondary Identification for Proof of Identity:
- State ID card
- Social Security Card
- Employee ID
- Voter registration card
- Medicare or other health card
- Student ID
- Selective service card
For applicants under the age of 16:
- Must be accompanied by proof of identity for parents
- Birth Certificate, Adoption Decree, or Court Order proving relationship between child and parents
- If both parents are not present, proof of sole guardianship or a notarized statement from the absent parent granting permission for a passport to be issued must be submitted
